Now that the 2011/12 season has finally come to an end, it's time for TWTD's annual questionnaire, your chance to look back in judgement on the campaign.

The questionnaire is quite long, so leave the odd question out if you can't decide on an answer, it doesn't apply to you, you're getting bored or you're a bit tired. Everything will be treated in the strictest confidence.

TWTD's questionnaire is the biggest annual poll carried out amongst Town supporters with more than 2,100 fans responding last year.
