The closest I can remember was 97-98 when Forest and Boro went straight back up. Sunderland lost in play off final to Charlton.
On a similar theme, Bolton, Palace and Barnsley all went down in that very 97-98 season having been promoted the year before, so we came very close to the 98-99 Premier League having exactly the same teams in it as 96-97. |  |

When Derby got promoted to the prem about a decade ago I think they beat West Brom to stop all three teams bouncing straight back, but I'm not not 100% on that.
