Not that I have any experience in this but I think it tends to be forehead (hard part of the head) on the nose (softest part). Plus the headbutter has momentum behind them so the energy's going in one direction.

Nope, one of the hardest parts of the body with momentum. That’s why they banned it from Muay Thai and that’s a sport where you’re praised for cutting people open with elbows!
Some of my best headbutts have hurt a bit, to be fair, but you get used to it. It's part of what makes a headbutt a headbutt, really, its the nonchalance of saying look I could just knock you out with a right cross but I'm going to just do it with my bonce because why not?

Many years ago at the Rose and Crown, I witnessed a headbutt where the headbutt-er split his head forehead open. A lot of blood started pouring down his face. It's a risky way to try and hurt someone!

I was in a band a few years back with a bass player who would head butt at the first hint of trouble, often that would be the the end of that. But once he got it badly wrong and the bloke replied “I think that hurt you more than it did me” before knocking him out. As someone who is rubbish at fighting and really doesn’t like it, it was a nervous few minutes.

It's also classed as use of an offensive weapon in courts because they consider the use of a headbutt to be a pre-meditated attack and they sentance you more accordingly as opposed to just right hooking someone.
