That is part of the image of being a 'character' *.
Paul Merton commented on it on HIGNFY, asking why Johnson had ruffled his hair before going on air. The 'make up' dept had just combed it. There has been frequent comments over the years mentioning similar actions, whereby he pulls his shirt out, performs a pantomime trip - or even when the Speaker reprimanded him for setting his phone to go over when he was in Parliament. Note how his tie is wrongly tied, so it is far too long. That is not by accident.

He has recognised that an awful lot of the not too bright are easily fooled, so he plays up to it. Watch how many times he is either photo'ed in some hi-viz jacket, or he is leering at the camera with two thumbs raised like some village idiot on a day out at the seaside.
