I think it was Dr John Campbell who said months ago we should be aspirating (pulling the needle back for 1 second to check for blood) when injecting and I note on the bbc headline story that step #2 of the findings is “It is injected into the muscle but sometimes leaks into the bloodstream”. Hmmm.
