Been pondering it this morning as a phrase. Doesn't really make sense does it?
If my interpretation is correct, in that it means someone who is a friend to you in your time of need, is a true friend, then it doesn't make sense does it? It suggests the need status is for that of the friend?
Or is it meant to mean if someone is in need, then they're a friend?
Unless when people say it they mean that they sh1t their pants in the middle of the night, and then woke up crying a bit later because they were desperate to suck on a pair of t1ts...

Most phrases are indeed pointless. For every inspirational quote, there is bound to be one that counteracts it.
"If at first you don't succeed, try try again" vs "Insanity is doing the same thing over and over and expecting different results."

"Actions speak louder than words." vs "The pen is mightier than the sword."
"The best things in life are free" vs "You get what you pay for."

"Oven ready deal" vs "Im making it up as I go along"
I am very fun at parties.
I vaguely recall there used to be a website where you could enter such phrases to work out which was correct, based on popular usage, I think it disappeared after someone tried testing a quote that started “what is right is not always popular…”
