Based on the EFL's own criteria, surely no club that's acting responsibly should find themselves in a position whereby they have such a large outbreak that they are unable to field any sort of team (u23 or otherwise).

We've shown it to be possible given this week's announcements re Covid; we've had it in the camp, but been able to limit the spread to just a handful of players. It can't be that hard.
