I believe incest laws have always been rooted in prohibitions set out in the Old Testament / Hebrew Bible, which (if memory serves, I couldn't cite the chapters or verses) specifically rule out sibling or parent/child relations, but make no mention of cousins.
All of which came long before any knowledge of genetic complications (although no doubt the consequences will have been observed).
