Has anyone got any particular recommendations (or companies to avoid) for putting a photo book together online?

Many are very similar quality - Photobox are fine.

DS Colour Labs are my go to for prints, not had a photo book from them though.

Not done one for years but I used Blurb when I did and the software was fairly decent.

I've used Snapfish a few times in the past, normally for gifts for people. They've always really liked them, or at least said they have, but then they would if its a gift I suppose. Quality has always seemed decent enough though and been printed correctly.

Used Snapfish over here extensively (family yearbooks. Holidays, kids sport seasons) and always got good quality books. They always offer good money off deals too.
