My local hospital trust. Sheesh.

At least it's out in the open. Maybe it'll make them cease their efforts to close down all the urgent care facilties in their largest population centre and move them 20 miles up the road so they can look after a small group of Welsh users who are supposed to have their own devolved healthcare system.

Likely a combination of poor practice, worse record keeping and communication with the families involved, a Trust with its priorities all in the wrong places and attempts to not actually do the right thing in the first place by admitting mistakes, investigating root causes and working to correct them in an any meaningful fashion. All probably exacerbated by a chronic shortage of staff.
