I don't think they do for every single game, just for certain milestones (debut, hat-trick, 10 caps, 25 caps, etc)

Yep. Everyone from schoolboy representative teams to England first team is presented with an actual cap for each game. I don’t think they ever wear them out though, mostly their heads are far too big.. | | | |
