smart thermostats come in two parts.... the smart bit, and the boiler coupling.... boilers are all simple, they basically just have demand switches.... on a combi, that is either the thermostat/controller demanding heat for radiators, or the hot water flow demanding heat.
An old fashioned thermostat just had a wired connection from wherever you installed it, to the boiler. Smart thermostats have a control link, which provides the "signal" to the boiler, in the same way that your old timer did. But they also have the thermostat and control unit, which is wirelessly connected to the control link. so when the "smart" bit works out that your heating should be on, it sends a signal to the control link, which will trigger the demand switch on your boiler.
For example, a HIVE system has 3 parts
Hive Hub - This manages the wireless links and the link to the outside world, its essentially a router for hive products, and will need to be connected to your router
Hive receiver - This is the control link that connects to your boiler
Hive Thermostat - this is the controller that you put in your house

In short, the control link is specific to your smart system, rather than your boiler, which only needs a simple on/off signal.

I have a Combi boiler, a lot of these smart thermostats say they control the hot water?
the only way I have ever controlled the hot water is by turning a tap on. I assume installing something like this wouldn't override that. But just be another way of turning the hot water on. (not sure why you would want the hot water on if you wernt running a tap / shower)

A lot of these systems CAN control heating AND hot water, but that is for systems that have a hot water tank, rather than combi boilers. Yours will just control the heating....

Our boiler is a combi boiler so the hot water is on demand the same as yours.
The hive controller can control your hot water, but we don't use that feature. As far as the Hive is concerned our hot water is always on (i.e. it has power available), we let the boiler deal with the actual heating of the water on demand.

We had an old gas boiler that we fitted a Nest Smartmeter to (and also smart front doorbell on the same system). Has saved us a lot: especially noticed that kettles overfilled, tumble drier used too much/not filled when used, down to leaving too many things on standby and lowering the thermostat 1 degree. Ot has been a very useful exercise coming into the next few years. Viva la Renewable Energy Revolucion is all I can say.
