Yeah, Montenegro ranked 72nd and no-one below the top 50 gets more than 2 visa points (of required 15). La Liga 2 is a Band 4 competition (6 points maximum for appearances, 6 points for club status). So even if he'd played in 90% of games for both Sporting Gijon and Montenegro, he'd still be short of points for a transfer to England.

Percentage of games played for which the player was available. So, for Band 4 you get 6 if you played in 90% of games, then a sliding scale going down every 10%.

I think the point there is that it is only the Spanish second division which doesn't gain many visa points.

no, there's a "reference period" which isn't explained very clearly in the document, but definitely doesn't go back more than one season.
