For those of you who have requested one but haven't heard anything, it may well be worthwhile speaking to Spotify via their online chat function.

I did so earlier in the week and asked (as per some comments on Spotify's community forum) for the matter to be escalated. This resulted in a code being manually generated for me and sent over by email.
