As a professional tradesman I think he should have known to take time and effort in identifying and using proper access equipment

Mind you, a fair few p&ds I know are very lax about their risk assessments on Working At Height

Yes very sad that somebody has died. Re your second point. They just look so tatty. If you go to the States the stars and stripes are flown on public buildings and many homes but they always look pristine and not out of place. These Union flags on lamposts look awful.
