Has anyone ever seen such a stark contrast in a team's form? I know Man City's bad run this season was quite something but to have 10 wins from 11 and then 0 from 13 is a bit mad.

I remember Hull in their first season in the Prem under Phil Brown got off to a great start
Talking of Hull, this was their form in 2019/20 when they were relegated from the Championship:

They sold both Jarrod Bowen and Kamil Grosicki on deadline day in January. That was the year they lost 8–0 at Wigan (7–0 down at HT!) when football started up again (PL and Championship) after the pandemic had put a stop to things.
