We did win one last season at Reading, which everyone seems to have forgotten!
Here’s our overall record. 28.6% success, though it doesn’t include a Full Members Cup one against Luton which we won in the early 90s and does include a load of LDV Vans nonsense. Take those out and our win % in meaningful cups is even worse - only 18.2%.
Missing a League Cup shootout loss against premier league Chelsea in 98. Was at this and remember it being a cracking game. Petta hit the post in extra time. Ruud Gullit played for them.

How many penalty shoot contests have we been involved in over last 25 / 30 years anyhow. There can't be too many instances, least not in real high profile games. Sheffield United '97 would seem to stand out as most damaging, at least with what was at stake and after a long hard season to miss out. To this day convinced we would have beat Palace in the Final game (believe we won 4 - 1 in a Cup game earlier that season) and that was a damn good team Burley was building around that time.
There was a high profile game with Liverpool some years later in the League Cup which believe was at Anfield and we held them over 120 minutes only to lose the resulting penalty contest. These are just two examples of standout games you can recall from years back where we missed out but other than the lesser ones already mentioned, not too many other instances really come to mind.

I'm sure i remember us winning one 2-1, which just about sums it up. (v Luton in some random cup back in the early 90's?)
Apart from that, I bet our overall record (certainly post 1983) is abysmal.
We beat Luton in the old ZDS Data Systems Cup in 91 (I think - and I also seem to remember we lost on pens to Chelsea in the next round?). We then didn't win another shootout until Shrewsbury in the League Cup in 2009 (perhaps Keane's one achievement with us!). And certainly then our penalty record was worse than even England's. Marginally better recently if you count the devalued pizza cup and it's 'no draws in the group stages' - I even finally saw us win one at Reading in the league cup last season too!
However, I think we must be about the only team whose fans would scream in the second half of extra time NOT to try holding on for penalties in a really tricky away European tie as we did at Liberec back in 2002. Even then we knew!
Still - we were still very unlucky last night that Forest hit such immaculate penalties as four of ours were also near perfect and Palmer guessed right two or three times where anything less than the corner would have seen a save.
