Had a fella a few weeks ago who insisted on being looked after by 'an English nurse'.
So, as there was nothing doing and there was two of us on, I looked after him all night.

Hmmmm, in that case, .....remember line the lower three knuckles up inline with the wrist/radius/ulna or go for a palm strike. Hit upwards and into the lower mandible.
With both stikes avoid making contact with the mouth, just in case you make direct contact with their teeth.
